Musculoskeletal ultrasound has become a first-line imaging modality for the evaluation of ankle pathology. It provides real-time, high-resolution, and dynamic assessment that complements, and in specific clinical contexts, may surpass static cross-sectional imaging. This lecture, structured as a practical workshop, introduces a systematic 360° ultrasound scanning protocol for the ankle. This standardized approach ensures a comprehensive evaluation of osseous surfaces, articular joints, ligaments, tendons, and peripheral nerves.

The presentation emphasizes diagnostic applications through case-based demonstrations, covering key pathologies: anterior talofibular ligament tears in acute inversion sprains, tibialis posterior tendon dysfunction in medial ankle pain, sinus tarsi syndrome, tibial nerve entrapment within the tarsal tunnel among other pathologies. To optimize diagnostic accuracy and reduce operator variability, the session highlights dynamic assessment techniques, power Doppler utilization for detecting neovascularization, and strategic probe positioning.

Beyond diagnostic imaging, the lecture delivers a structured guide to ultrasound-guided interventions for common ankle conditions. Step-by-step protocols are detailed for tibiotalar joint, subtalar joint, and peroneal tendon sheath injections, as well as for plantar fascia procedures. Focus is placed on critical sonoanatomy, optimal needle visualization, pertinent safety considerations, and the documented accuracy advantages over traditional landmark-based techniques.
